Nurse Practitioner – Primary Care at Home

The opportunity
The PCAH service has a strong focus on chronic disease management and provides healthcare to vulnerable clients who experience significant barriers to accessing health services, within Metro Perth Western Australia. It provides a holistic person-centred service that meets the physical, social, psychological and emotional needs of the individual, while providing an evidenced based best practice aiming to close the healthcare gaps within the community.
We are currently seeking a Nurse Practitioner who is comfortable at providing a service autonomously to clients within their homes in the community. Are you available 8 days per fortnight, between Monday – Friday, 8.00am – 4.00pm.
Key responsibilities
As a Nurse Practitioner you will engage with consumers with chronic diseases who experience significant barriers to accessing health services, thereby aiming to close healthcare gaps within our community Empowering healthcare consumers to be partners in managing their own health and that of the community. You will work in partnership with community support services to provide holistic care for vulnerable clients.
About you
The ideal candidate for this role will meet the following criteria:
- A current Nurse Practitioner Registration
- Demonstrated experience and / or postgraduate qualification in area of designated practice, for example Primary care /Chronic Disease Management.
- Well – developed knowledge regarding community resources available to enhance client quality of life.
- Demonstrated advance clinical knowledge, skills and extended practice relevant to position.
- Proven commitment to the development of learning, teaching and research orientated work, within a collaborative multi-disciplinary environment.
- Ability to undertake and evaluate nursing research and integrate evidence-based research to practice.
- A Current Australian drivers licence
Under the Aged Care act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement.
